Whipple Multimodal PSH Orders

Preop

Acetaminophen 1000 mg

Gabapentin 600 mg

Oxycodone 10 mg

Intraop

Magnesium 30 mg/kg IV bolus, then 10 mg/kg/h IV (intermittent bolus or infusion)

Ketamine 0.3 mg/kg IV bolus, then 0.15 mg/kg/h IV (intermittent bolus or infusion)

Ibuprofen 10 mg/kg IV bolus

PACU

Acetaminophen IV 1000g (when >6 hours after previous dose)

Continue epidural infusion, call APS for questions

Hydromorphone 0.4 mg IV q15min for pain 3-5

Hydromorphone 0.8 mg IV q15min for pain 6-8

Fentanyl 100 mcg IV q10min for pain 9-10

Preop

Lidocaine 1 mg/kg IV bolus, then 1 mg/kg/h infusion (start intraop, cont. 24 hours)

Ketamine 0.3 mg/kg IV bolus, then 0.15 mg/kg/h IV (intermittent bolus or infusion)

PACU

Continue IV Lidocaine 1mg/kg/h infusion (started intraop)
